Case Report: Simultaneous penetrating keratoplasty with autologous simple limbal epithelial transplantation as an alternative to keratoprosthesis

Introduction and importance: This case report highlights the multidisciplinary approach required to achieve successful anatomical functional outcomes, in an eye with total limbal stem cell deficiency (LSCD) associated underlying corneal scarring thinning. Presentation of case: A 59-year-old gentleman had poor visual recovery right (RE) following accidental carbide blast, 1-year before presenting us. The acuity was counting fingers clinical examination revealed cicatricial entropion involving upper eyelid, LSCD, a central descemetocele cataract RE. Prior ocular surface reconstruction, correction performed. Three months later, penetrating keratoplasty combined surgery intraocular lens implantation (penetrating (PK) triple), autologous simple epithelial transplantation (SLET) 20/100, 18 after surgery, clear well-epithelized graft stable surface. Discussion: LSCD is caused by decrease population /or function cells. Limbal (LSCT) warranted eyes LSCD. In coexisting scarring, LSCT alone may be inadequate restore vision. These require simultaneous or sequential lamellar full-thickness for rehabilitation. Though, existing literature favors approach, where performed first followed transplantation, under certain circumstances such as thin cornea like our case, have optimal outcomes. Conclusion: Combining SLET PK can rehabilitation unilateral thinning. Corneal survival prolonged if adnexal comorbidities are addressed any surgical intervention planned adequate time interval allowed inflammation subside.
